Here’s how you can start taking advantage of your membership:

1. Download our communication app
Click the button above or click here to sign up for Spruce, our secure messaging and telehealth platform. Now you are ready to connect with me via text/virtual visits at your convenience!  FAQ form.

2. Schedule an appointment
You may request an appointment by:

3. Medical History Form:
Once you schedule your first appointment, you will receive an email requesting you complete your medical history form electronically prior to your visit.

If you prefer a paper copy you can use this Medical History Form.

4. Past Medical Records:
Please fill out a Records Request Form for us to obtain your past medical records, please submit the Record Request Form to your past provider.

5. Access your health information:
After your first visit, you will receive a link to access the Elation patient portal. This portal is an easy way to access your medical reocrd with us.

6. New Patient Handout

Please read over our New Patient Handout. It highlights how Blue Spruce works and some helpful information to make being a Blue Spruce Member easy and convenient.
